Supports Coordination
We provide a full array of required supports coordination services
and the delivery of person-centered planning for persons with a
wide array of disabilities in varied work and residential situations.
Our Supports Coordinators establish a close relationship with you,
your family, care providers, and natural supports (friends, neighbors,
etc.) and respond as appropriate to changes in your life.
We work with you to ensure the responsible delivery of service
and goals along with family and others.
We are experienced and able to complete all of the assessments
required by the MCPNs, Michigan Comprehensive Provider Networks.
Our own Person Centered Profile has been commended by auditing bodies
for its comprehensiveness and its focus on the person.
Your goals are developed and written according to your desires,
wishes, health, safety, needs, and strengths.
Here are some of the ways we can help you:
Assisting in all matters of entitlements, such
as Social Security, Medicaid, SSI, Medicare, Home Help Services,
Section 8 housing, food stamps, wage records, spend-down, medical
determinations, applications, and appeals.
Linking you with residential, social, recreational, respite,
senior, transportation, medical, dental, and religious services.
Supports Coordinators understand and will assist you with
employment, including self employment, individual job placement,
group employment, and our own Resource Center
based employment.

We will encourage and support you to take responsibility, to take
charge of your life, and to develop activities and friends in your
community.

your Supports Coordinator know. It all begins with your Person Centered
plan while working closely with your MCPN, supports coordinator,
family, and others.
